⚽ F3 Soccer Academy – Parent Policy & Requirements Form

Welcome to F3 Soccer! To ensure a safe, fun, and organized experience for every player, please read the following policies and guidelines carefully before enrolling your child.

✅ Program Policies

🛡️ Class Safety Guidelines

Medical Info: Please provide all relevant medical information during the online registration process.

Adult Supervision Required: This is not a drop-off program—a parent or guardian must stay with the child throughout the class.

No Food or Snacks: Only water is permitted during sessions.

Eye Protection: Children who wear glasses must use shatterproof eyewear.

Footwear: Outdoors on grass: Cleats allowed

Shin Guards: Recommended for children ages 4+, but not required.

Field Access: Only registered participants are permitted on the field or in the gym during class. Siblings and non-participating children must remain off the playing area and be supervised by an adult. For safety and to minimize distractions, parents are not allowed on the field during the session for players ages 4 and up.

💳 Refund & Attendance Policies

Refunds:

• $120 refund available before the 2nd paid session of the season.

Withdrawals:

• Must notify F3 Soccer at least 24 hours before Week 2 to officially withdraw.

Missed Classes:

• No refunds for missed classes.

• Up to 1 make-up classes allowed per season, except during the first and final week.

• Make-ups must be scheduled with F3 Soccer in advance.

Cancellations:

• Classes are rarely canceled due to weather (light rain usually does not cancel a session).

• In case of cancellation, parents will be notified by text or email.

Enrollment Requirements:

• All outstanding balances must be paid to register for a new season.

✅ Soccer Ball— Please bring your own size-appropriate soccer ball.

✅ Water— Stay hydrated and energized!

✅ Official F3 Soccer Jersey Top— Available for purchase.

✅ Comfortable Clothing— Shorts, leggings, or sweatpants work best.

✅ Proper Footwear— Outdoors (on grass): Soccer cleats are allowed

1. Can my child try a class before enrolling?

— Yes! We offer trial classes so your child can experience the program firsthand. Just let us know if you’re interested, and we’ll be happy to get you scheduled!

2. Are there discounts for families?

— Yes! Enroll two or more children and receive a $10 discount.

3. Is this a drop-off program?

— No. All children must be supervised by a parent or guardian throughout class.

4. Where are classes held?

— Lincoln Orens Middle School (Island Park)

Saturdays: 10 AM — 11 AM (Ages 2-5), and 11 AM — 12 PM (Ages 6-8)

LIABILITY WAIVER

In consideration of the risk of injury while participating in soccer training (the “Activity”) and as a condition for the right to participate, I, on behalf of myself and my child/participant, hereby knowingly and voluntarily enter into this Waiver and Release of Liability. I understand and acknowledge that participation in the Activity involves certain inherent risks, including but not limited to physical or psychological injury, illness, paralysis, disability (temporary or permanent), emotional or economic loss, property damage, or death. These risks may arise from my own actions or inactions, the actions or inactions of others, the conditions of the facility, weather, equipment, or travel to and from the Activity. By signing below, I voluntarily assume all such risks, known and unknown, and fully accept personal responsibility for any injury, loss, or damage that may occur during or as a result of participation in this Activity, including travel to and from any related events. I hereby release, discharge, and hold harmless F3 Soccer of USA, LLC (DBA F3 Soccer Academy), its staff, coaches, volunteers, affiliates, successors, and assigns from any and all claims, liabilities, or causes of action arising out of or in connection with my or my child’s participation in the Activity. This release includes, but is not limited to, claims for negligence to the fullest extent permitted by law. I agree to indemnify and hold harmless F3 Soccer of USA, LLC from any claims, suits, actions, or expenses (including attorney’s fees) arising out of my or my child’s participation in the Activity, including any damage caused by our willful actions, negligence, or recklessness. I understand that this Activity may test physical and mental limits and may involve risks such as: • Uneven terrain or facility hazards • Weather-related conditions • Hydration or temperature issues • Participant behavior and equipment use • Vehicular or pedestrian traffic near the facility In the event that medical care is required, I agree to be financially responsible for any associated costs and understand that it is my responsibility to carry personal health insurance. Should any damage occur to the facility or equipment due to the actions of myself or my child, I acknowledge financial responsibility for the cost of repairs or replacement. ⸻ 📸 Media Release I give permission to F3 Soccer of USA, LLC (DBA F3 Soccer Academy) to photograph or video record my child during participation in classes, games, or other events. I understand that these images may be used for promotional, marketing, or educational purposes, including (but not limited to) brochures, flyers, social media, website content, or print material. I waive any rights to compensation or ownership of such images and understand that names will not be used without additional consent.

ACKNOWLEDGEMENT & SIGNATURE

By signing below and submitting this form, I confirm that I have read, understood, and voluntarily agree to the terms of the Waiver and Release of Liability, including the Media Release section. I understand that this agreement includes important legal terms and may limit certain rights.
